Purchase object: medical services, in order to investigate the state of health of employees by providing the following services:

a) occupational medicine services according to the labor code Law 53/2003

b) specialized medical services and paraclinical investigations upon the recommendation of the occupational medicine doctor, for employees of ADR Nord-West

Purpose of purchase: medical services, in order to investigate the state of health of employees

Period: April 2020 – December 2020

Estimated value/person: 120 lei/employee

Occupational medicine and medical services for 170 employees - 20,400 lei, between April 2020 and December 2020.

Purchase object description (technical specifications): The requested medical services are:

a) occupational medicine services according to the labor code Law 53/2003 and

b) specialized medical services and paraclinical investigations on the recommendation of the occupational medicine doctor, for employees of ADR Nord-West:

                -One set/year of laboratory analyzes (which can be performed together with the periodic control): complete blood count, TGO, TGP, serum urea, creatinine, blood glucose, LDL cholesterol and HDL-C, triglycerides, calcium, magnesium, ESH, urine summary .

                -1 general ultrasound;

                -1 breast ultrasound for employees

                -1 EKG;

                -1 gynecological consultation + Pap test - for women;

                -1 PSA – for men;

                – 1 general ophthalmological examination;

                – A consultation of your choice from the existing specialties

Contract: YES

The financial offer will be drawn up in lei and will contain the costs grouped as follows:

• Cost of occupational health services/employee - according to point a).
• Cost of specialized medical services and paraclinical investigations - see point b).
• Total cost per employee

It will be mentioned if the prices offered are with or without VAT, or if the services are exempt from VAT.

Award criteria: the lowest price / total offer, subject to the full fulfillment of the requirements and technical specifications.
